Missed appointment with dermatoligist?

my doctor referred me for a skin condition and i missed the appointment as i forgot the doctors had my old address so i was waiting for appointment to come through and it would of went my old address. it was through nhs choose and book which wont let me log in now as it says my appointment takes place in the past. do i have to visit my doctor again to get a new appointment? will he refer me again after ive missed it, I dont want to go back the docs incase he has a cob on with me fo missing it and thinks im a time waster but im not that type of person it was a genuine mistake but now im gutted because i really need to see a specialist as its getting worse
